Improving Care For Etobicoke Patients
Description: On February 10, at LAMP Community Health Centre, Etobicoke-Lakeshore MPP Peter Milczyn announced how Ontario is working together with local healthcare teams in south Etobicoke to provide better, more personalized care for the five percent of patients with the highest needs, often with multiple, complex conditions, and who account for two-thirds of the province's healthcare dollars. The South Etobicoke Health Link will provide patients with care through individual plans and coordinated support from a tightly knit team of providers that could include doctors, nurses, specialists, hospitals, home care, long-term care, and community agencies. It
was also announced that Ontario has established the Complex Patient Community Project in Etobicoke that will enable a close follow-up of complex conditions and ensure safe and proper care in the community in an effort to prevent emergency visits or extended re-admissions to hospital.
